First Visit

Before the appointment:

Please fill out and submit your health history form, HIPPA form, and the release form if you are transferring over any previous X-rays or information from your previous dental office.

When to arrive:

Please arrive 15 minutes before your appointed time.

What to bring:

Please bring your dental insurance information, a valid ID, and a list of medications.

What to expect:

Nervous? You are not alone! This is why we treat all our patients with gentle care. At the first appointment, the dental assistant or hygienist will take digital X-rays. Then the doctor will read the X-rays and perform the clinical exam. This involves evaluating much more than just the teeth! We will look at the gums and muscles of the mouth too. The doctor will then talk with you and create an individualized treatment plan for you.
